Taking care of your teeth and gums is important to your overall wellness. With dental insurance through Delta Dental of California, you have coverage for preventive care and dental treatment.

Overview

Lineage offers two dental plans with different benefit levels and costs, so you can choose the one that works best for you. The Premium Plus Dental Plan has higher benefits and a lower deductible, and the Premium Dental Plan is a lower-cost plan with a slightly higher deductible.

You can enroll in dental coverage as a new hire, during Open Enrollment or if you have a Qualifying Life Event (QLE).

* Plan reimbursement for Delta Dental Premier dentists is based on their Premier contracted fees. Reimbursement for Non-Participating dentists is equal to the 80th percentile of reasonable and customary charges. When services are received by a Non-Participating dentist, any amounts exceeding the 80th percentile are subject to balance billing and payable by member.

*** Orthodontia has a $2,000 lifetime benefit maximum for adults and children in the Premium Plus plan and a $1,000 lifetime benefit maximum for children only in the Premium plan; both are disbursed in conjunction with the treatment plan rather than paid out upfront.

How much does dental coverage cost?

You pay for dental coverage through pre-tax deductions from your paycheck. The amount you pay will depend on which plan you choose and whether you cover just yourself or family members, too. You can view your plan costs in Workday.

Request a pretreatment estimate
