To help you keep life in focus, Lineage offers you vision coverage through Vision Service Plan (VSP) to help pay for eye exams and vision correction.

Overview

You have two options for vision coverage:

  • The Basic Plan is a lower-cost vision plan
  • The Enhanced Plan has expanded benefits, including higher allowances for vision correction materials

You can enroll in vision coverage as a new hire, during Open Enrollment, or if you have a Qualifying Life Event (QLE).

KidsCare
  • 2 eye exams per year, $10 per exam
  • Frames every year
  • Additional lenses covered with an Rx change within same year, $25 for lenses
Glasses and Sunglasses
  • 30% savings on additional glasses and sunglasses (including lens enhancements, from the same VSP provider on the same day as your exam)
  • Or 20% savings (from any VSP provider within 12 months of your exam)
Retinal Screening
  • No more than a $39 copay (on routine retinal screening as an enhancement to an exam)
Laser Vision Correction
  • Average 15% off the regular price
  • Or 5% off the promotional price
  • Discounts only available from contracted facilities
  • After surgery, use your frame allowance (if eligible) for sunglasses from any VSP doctor

How much does vision coverage cost?

You pay for vision coverage through before-tax premiums from your paycheck. The amount you pay will depend on which plan you choose and whether you cover just yourself or family members, too. You can view your plan costs on the Workday website.

Find a network provider

When you use one of the providers in the VSP’s extensive network, you receive a higher level of coverage and are only required to pay a copay at the time of service. Visit the VSP website or call Member Services at 1-800-877-7195 to find an in-network vision care provider near you.
