Voluntary Benefits
To help protect the financial wellbeing of your loved ones, Lineage provides company-paid life and long-term disability insurance at no cost to you. You can purchase additional life and short-term disability insurance as well as Whole Life Insurance with Long Term Care.
2025 Life, AD&D and Disability Insurance plans
Company-paid Life and Disability Insurance
Lineage provides Company-paid team member life and long-term disability insurance coverage of a specific amount that can help your loved ones by covering your final expenses and funeral costs, replacing lost income and more.
Optional Life and Disability Insurance
If you want additional coverage, you can buy optional life and short-term disability insurance for yourself and your family members beyond the Company-paid coverage through Unum.
Whole Life Insurance
Whole Life Insurance with Long Term Care, provided by Allstate, is a type of permanent life insurance you can purchase that lasts for your lifetime, as long as you pay your premiums. It’s also portable, meaning you can take it with you if you leave Lineage.
Supplemental Benefit Plans
While your primary medical coverage pays doctors and hospitals directly for costs related to your care, supplemental medical plans pay cash benefits directly to you — you can use this money however you want.

How much does supplemental medical coverage cost?
Supplemental medical coverage is relatively inexpensive. The amount you pay will depend on which plan(s) you choose and whether you cover just yourself or family members, too. In addition, critical illness insurance rates vary by age, tobacco use and the benefit level you select. If you enroll in Hospital Indemnity Insurance or Critical Illness Insurance and complete at least one health screening test on or after your coverage effective date, you can receive an annual benefit payment.
You pay the full cost of coverage through post-tax paycheck deductions. To see your premiums and enroll, log in to the Workday website.

You also can enroll in identity protection coverage and a legal plan if you choose.
Company-Paid Life and Disability Insurance
Your life and disability benefits are some of the most valuable benefits available to you. Often referred to as “survivor” benefits, they can help your loved ones by covering your final expenses and funeral costs, replacing lost income and more. These benefits are provided through Unum.
As a Lineage team member, you automatically receive at no cost to you:
- Basic life insurance of up to 2 x your base annual salary, up to plan limits
- AD&D insurance of up to 2 x your base annual salary, up to plan limits
- Long-term disability (LTD), which provides financial protection should you experience a serious illness or injury that prevents you from working for an extended time (more than 180 days)
What is AD&D insurance?
If you lose your life, sight, hearing, speech or use of your limb(s) in an accident, AD&D insurance provides additional benefits to help keep your family financially secure. AD&D benefits are paid as a percentage of your coverage amount — from 50% to 100% — depending on the type of loss.
How company-paid life insurance is taxed
The IRS requires you to pay taxes on the value of any company-paid life insurance coverage over $50,000. This is called “Imputed- GTL”, you’ll notice it on your pay statements and your W-2.
Optional Life and Disability Insurance
For financial protection beyond Lineage’s basic coverage, you can buy optional life, AD&D and disability insurance for yourself and your family members beyond the Company-paid coverage through Unum. You can enroll on the Workday website as a new hire, during Open Enrollment or if you have a Qualifying Life Event. You pay the full cost of this coverage, which is provided through Unum. You may have the opportunity to purchase coverage at the guaranteed issue (GI)* amount, or you can complete an evidence of insurability (EOI)** form to purchase coverage above the GI limit.
Short-Term Disability
- Lineage will replace a portion of your earnings if you are unable to work due to a non-work-related short-term illness or injury
- Your cost is only $2.87 per month ($0.66/week)
Please note: You must actively elect your STD coverage. You will not be automatically enrolled in coverage.

*What is guaranteed issue?
Guaranteed issue (GI) is the amount of coverage that you can purchase for yourself and your dependents without answering any medical questions. This opportunity is only available to you during your new hire/newly eligible enrollment or Open Enrollment if you’re already enrolled at an amount less than the GI limit. If you elect life insurance coverage above the GI amount, you will need to complete evidence of insurability within 120 days of enrollment. You will see a notification to complete your evidence of insurability in your Workday Tasks.

**Evidence of insurability
Supplemental life insurance for you and your spouse/partner over a certain benefit amount may require evidence of insurability (EOI), or proof of good health. HR Connect EOI works through a single sign-on (SSO) connection in Workday allowing you to complete your EOI form online.
If EOI is required, Workday will be set up to trigger an Alert or To-Do Notification for you to complete your Statement of Health via the Announcements section of the Workday Home Page. The Statement of Health is a set of formal questions Unum asks to establish Evidence of Insurability. Unum will contact you directly to collect any needed medical documentation. Coverage decisions will be communicated through Workday after the medical documentation is reviewed by Unum staff. You will receive more information after electing coverage if EOI is required.
Whole Life Insurance with Long Term Care
Whole Life Insurance can help provide financial security for life and its uncertainties. Have peace of mind and confidence knowing your loved ones are protected when you elect Whole Life coverage.
How it Works
Whole Life Insurance, provided by Allstate, is a type of permanent life insurance: unlike a term life policy, which ends after a specific amount of time, Whole Life lasts for your lifetime, as long as you pay your premiums. You can elect coverage of $25,000, $50,000, $75,000, $100,000, $125,000 or $150,000 without any required health questions or medical exams for those ages 18-70. Once you’ve chosen your level of coverage and enrolled, you may cancel but not increase or decrease your coverage in the future.
Whole Life coverage also includes three riders* that can provide additional benefits in specific circumstances:
- Accelerated Death Benefit for Long Term Care: a monthly advance of the death benefit while receiving qualified long-term care services after a 90-day elimination period when certified chronically ill
- Accelerated Death Benefit for Terminal Illness or Condition: an advance of the death benefit, up to 75% of the certificate face amount, when certified terminally ill
- Accelerated Death Benefit for Chronic Illness: a lump-sum advance of 100% of the death benefit when certified chronically ill
* The riders (above) have exclusions and limitations; may vary in availability by issue or termination age and may not be available to all covered dependents or in all states.

Name a Beneficiary
A beneficiary is an individual or entity that you’ve chosen to receive the death benefit of your Unum or Allstate insurance policy. You may choose one or more individuals, including a trust or your estate, as a beneficiary. Make sure to keep your beneficiary designations up to date so there’s no confusion about who gets your life insurance benefits when you pass away.

Submitting a Life/Death Claim
When submitting a claim, you will need to provide essential documentation to Unum and/or Allstate such as a death certificate, medical history, proof of identity and relevant policy details, to ensure a smooth review. If the claim is approved, Unum and/or Allstate will issue the death benefit payment to the designated beneficiaries, as outlined in the policy.
The easiest way to file a life claim is to email benefits@onelineage.com. The email should include the team member name and ID of who owns the policy, who has passed away (team member or dependent name) and the contact number for the beneficiary. A benefits team member will then reach out to the family to discuss the claims process.
Accident Insurance
You can’t prevent all accidents, but it’s possible to get financial help during your recovery.
How it Works
Accident Insurance through Voya can help you offset the costs associated with a covered accident. It provides fixed benefits for events tied to an on or off the job accident. The benefits can be used for any purpose you choose, including things like health insurance deductibles, copays, child care or home health care.
Please Note: This is a limited benefit policy and does not satisfy the requirement of minimum essential coverage under the Affordable Care Act.
Accident Insurance provides payment for a wide variety of covered accidents and related medical treatment, including things like:
- Emergency room and urgent care facility visits
- Ambulance transportation
- Hospital admission and confinement
- Critical care unit confinement
- Inpatient and outpatient surgeries
- Medical testing services including X-rays, MRIs and CT scans
- Medical equipment and prosthetic devices
- Physical, occupational or speech therapy

Accident Insurance also includes a Sports Accident Benefit, which pays an additional 25% of the Accident Hospital Care, Accident Care or Common Injuries benefit amount, up to a maximum benefit of $1,000, if the covered accident is the result of an organized sporting activity.
Coverage Options
You can enroll yourself, your spouse and your children in Accident Insurance at two different levels of coverage. The High Plan provides greater payments in case of accidents than the Low Plan.

Hospital Indemnity Insurance
Hospital stays can be pricey and are often unexpected. When in the hospital, it’s likely you’ll need treatments, tests and therapy to get up and about. Hospital Indemnity Insurance from Voya can help you pay for expenses that would usually come out of your own pocket.
How It Works
A flat amount is usually paid to you for an eligible hospital admission and a per-day amount for your entire hospital stay. The payment is made directly to you in addition to any other insurance you may have. Plus, you can use the payment as you choose: you can offset copays and other charges from your hospital stay, or cover everyday expenses like groceries, childcare or utilities.

You can enroll yourself, your spouse and your children in Hospital Indemnity Insurance at two different levels of coverage. If you have Child coverage and have a new baby during the plan year, that newborn will be covered under your plan.

Wellness benefit
If you enroll in Hospital Indemnity Insurance and complete at least one health screening test on or after your coverage effective date, you can receive an annual benefit payment of $50 whether or not there is any out-of-pocket cost to you. You can also receive a $50 benefit for your spouse and a $25 benefit per child (up to $100) if they are covered under the Wellness Benefit and complete a health screening. Qualifying health screening tests include routine colonoscopies, eye exams, cholesterol tests and many others.

Critical Illness Insurance
Critical illness insurance protects against the financial impact of certain health conditions such as a heart attack, cancer or stroke. This can help reduce financial stress during recovery.
How It Works
With Voya’s Critical Illness Insurance, you can receive a lumpsum benefit payment if you are diagnosed with a covered illness under the policy. Plus, the plan includes coverage for additional critical illnesses and for certain conditions that may naturally recur. Payment is made directly to you and you can use it however you like.
Critical illness insurance benefits
You can choose a benefit amount of $10,000 or $20,000 during Open Enrollment or as a new hire. Benefits are paid upon diagnosis of a covered illness or condition and may be paid more than once if the illness recurs or if a different diagnosis is made. Examples include:

* A sudden cardiac arrest is not in itself considered a heart attack.
** Listed in the certificate of coverage as “major organ transplant,” which means the irreversible failure of your heart, lung, pancreas, entire kidney or liver, or any combination thereof, determined by a physician specialized in care of the involved organ.

Legal Plan
Legal matters, both planned and unplanned, are part of life. Enrolling in a Legal Plan can bring financial and emotional peace of mind, knowing you will be covered in case of expected and unexpected legal events. The MetLife Legal Plan offers economical access to attorneys for a wide range of legal services, such as will preparation, real estate and family law.
How It Works
The Legal Plan through MetLife provides you, your spouse and dependents with fully covered legal services from attorneys experienced in estate planning documents, civil suits, adoption, identity theft, divorce and custody issues, reproductive services and much more. The cost of the plan will be deducted from your paycheck and you can access legal help from a wide network of attorneys with no deductibles, copays, claim forms or usage limits.
You can consult with a network attorney online, on the phone or in person as best fits your schedule. You can also use an out-of-network attorney and be reimbursed for covered services according to a set fee schedule.*
