Your new hire checklist

Follow the steps in this checklist to make a great start at Lineage:

  • Get to know our company by familiarizing yourself with the Team Member Handbook, learning about our culture and values and connecting with colleagues.
  • Meet with your manager to discuss goals and professional development opportunities.
  • Learn about your benefit options by exploring this website and think about your coverage needs.
  • Review the eligibility requirements. If you plan to include dependents, documentation will be required in order for Lineage to verify them.
  • Enroll in your health, voluntary and supplemental benefits. Please note: The enrollment event won’t be launched in Workday for you to start enrolling in benefits until the 7th day of your employment with Lineage. Your elections are due within 15 days of your hire date. Learn how to enroll.
  • Save for the future by enrolling in the Lineage Retirement Savings Plan to prepare for retirement and provide beneficiary information for your 401(k) Plan account.

Benefits Eligibility

Review the information below to confirm your eligibility before enrolling in benefits.

Health, voluntary and supplemental benefits

If you are a regular team member scheduled to work 30 or more hours per week, you are eligible to participate in Lineage’s benefits. You may also cover your eligible dependents under Lineage’s core health benefits and supplemental plans. As a new Lineage team member, your elections are due within 15 days of your date of hire* and your coverage will become effective on the first day of the month following 30 days of employment.

Your eligible dependents include:

  • Your legally married spouse (same or opposite gender)
  • Your registered domestic partner (required documentation, such as a marriage or domestic partnership certificate or a joint tax return, will come from Dependent Verification Services)
  • Your children under the age of 26, including natural children, stepchildren, legally adopted children, children for whom you are the legal guardian (choose Child as the dependent type in Workday if adding a dependent due to guardianship), foster children and children for whom you are legally responsible to provide health coverage under a Qualified Medical Child Support Order (QMCSO)
  • Your disabled children 26 years and over if unmarried, incapable of self-support, dependent on you for primary support and the disability occurred before the age of 26 (contact benefits@onelineage.com for the required Disability Statement)

If you and your spouse are both Lineage team members and are both eligible for benefits, you may each be enrolled as an individual team member or be covered as a dependent of the other person, but not both. In addition, only one parent may enroll children as a dependent to their plan.

*Please note: The enrollment event won't be launched in Workday for you to start enrolling in benefits until the 7th day of your employment with Lineage.

Dual Year Enrollment

As a new hire, you may need to enroll in benefits for both the 2025 and 2025/2026 plan years. The below information explains how it works, and you will receive additional information about enrolling in benefits as a new hire.

If you were hired prior to May 1st, 2025:

  • Step 1: Enroll in your 2025 benefits within 15 days of your hire date
  • Step 2: Review, confirm and adjust your benefits during 2025/2026 Open Enrollment

If you were hired on or after May 1st, 2025:

  • Enroll in your new hire benefits from May 19 – June 6, 2025

If you are making 2025 benefit changes due to a Qualifying Life Event (QLE):

  • Step 1: Report the Qualifying Life Event and make benefits changes within 30 days of your life event
  • Step 2: Review, confirm and adjust your 2025/2026 benefits during Open Enrollment

401(k) Plan

Team members become eligible to contribute to the 401(k) on the first of the month following 30 days of continued employment. Team members become eligible for the Lineage match on the first of the month after they’ve worked at Lineage for 6 months.

  1. Getting started with the 401(k): Once you become eligible, you’ll be contacted by Vanguard directly to set up your account. If you’ve been with Lineage for more than 30 days and are not already using the 401(k) savings plan, contact Vanguard.
  2. Once your account is created, you can choose how much of your paycheck to set aside. You can choose any amount that works for your family up to tax limits, but remember, a contribution of 5% or more ensures you receive the maximum match from Lineage.

After six months, Lineage will:

  • Match 100% of your contributions up to 3% of pay
  • Match 50% of your contributions for the next 2% of pay

This means that if you put at least 5% of your pay towards your retirement savings, Lineage will contribute an amount equal to another 4% of your pay. Plus, our plan is a safe harbor 401(k), which means your contributions are immediately yours with no vesting period. This ensures that you have full access to your money right away, providing you with peace of mind. Don’t leave money on the table – get your 401(k) going today!

You may change your contribution rate and investment elections at any time by logging in to the Vanguard website or calling 1-800-523-1188, available Monday - Friday 7:30 AM to 8:00 PM CT. For more information about the 401(k) plan, please visit the retirement plan page.

Important!

Make sure to provide beneficiary information for your 401(k) Plan account. In the event of your death, your 401(k) balance will go to your beneficiaries. Naming your beneficiary ensures the money goes to the right person. You can designate more than one beneficiary or name a trust, charity or estate to receive the benefit.

Please Note: Vanguard beneficiaries are noted within Vanguard only and are not housed in the Workday system.

How to Enroll

Learn how to enroll in health and income protection benefits such as medical, dental, vision and supplemental life insurance coverage.

Health, voluntary and supplemental benefits

You must enroll within 15 days of your date of hire to have coverage this year. Your coverage will become effective on the first day of the month following 30 days of employment.

Please note: You are not able to begin the enrollment process until 7 days after your hire date. The enrollment event won’t be launched in Workday for you to start enrolling in benefits until the 7th day of your employment with Lineage.

Enrolling is easy!

When you’re ready to enroll, visit your Workday website home page, then click into “My Tasks” in the top right corner of the screen. From there, you’ll see that you have a “Benefit Change- Hire” task assigned to you. Click on “Let’s get started!”

You will first be asked if you are a tobacco user — this information will be used to create your personalized rates for certain insurances.

Once you are in the Enrollment task, you’ll see your benefit choices as tiles on the screen. To enroll in a benefit, simply click on the associated tile and you’ll be guided through the enrollment process, which may include enrolling dependents or making additional selections. After you finish, you will be returned to the main screen with all of your benefit options.

You’ll need to enroll in each benefit individually, so select the tile for each benefit you’d like to enroll in and complete the process. To complete your enrollment, keep two things in mind:

  • You need to actively elect Short-Term Disability (STD) coverage. You will not be automatically enrolled. When enrolling in STD, you must select both “Employer” and “Employee” contributions.
  • You do not need to actively elect Life & AD&D insurance or Long-Term Disability (LTD) to receive benefits. These are fully paid for by Lineage. However, you will need to assign beneficiaries for Basic Life & AD&D insurance to be able to complete your enrollment in this coverage.

Once you have enrolled in all your individual benefit choices, you can select the “Review and Sign” button on the bottom left of the screen to complete enrollment.

Remember!

  • Be sure to provide the necessary dependent information (name, birth date, SSN) to enroll eligible family members.
  • Make sure you’ve provided beneficiary information for your life insurance in Workday and the 401(k) plan on the Vanguard website.

Do I need to enroll?

New hire elections are due within 15 days of your date of hire* and your coverage will become effective on the first day of the month following 30 days of employment. You MUST take action within 15 days of your date of hire to have the benefits you want in 2025. If you don’t enroll, you will only receive automatic company-paid benefits, such as basic life, AD&D and long-term disability (LTD).

* Please note: The enrollment event won’t be launched in Workday for you to start enrolling in benefits until the 7th day of your employment with Lineage.

Don't miss out

If you don’t enroll within 15 days of your date of hire, you can’t make benefit elections until the next annual Open Enrollment period, unless you experience a Qualifying Life Event (QLE).

Dependent Verification Audit

To keep your health care costs low and to meet legal requirements, Dependent Verification Services will reach out to verify your family members’ eligibility. You will be asked to provide documentation such as marriage and/or birth certificates. A domestic partner may be required to provide a domestic partnership certificate or a joint tax return.

If backup documentation is not provided by the deadline provided, your dependent(s) will be removed from coverage and premiums will not be refunded.
