QuestSelect™ through Quest Diagnostics is a lab-testing benefit that saves you money — and it’s free year-round to anyone who is enrolled in a UnitedHealthcare plan. Your Quest Diagnostics card will give you and your eligible dependents the benefit of reduced out-of-pocket costs on covered outpatient lab testing ordered by your physician. To take advantage of savings, you must tell your healthcare provider and show your card any time a lab test is ordered.

Visit MyQuestSignup.com to activate your patient portal. Then, use the portal to view lab results, find Patient Service Centers, make appointments, track your health history and more.

How to use your Quest Diagnostics card:

• At your appointment, show your card and ask your doctor to send your lab work to Quest (Using QuestSelect™ is optional – if you don’t use your card, your regular benefits will apply)
• If your doctor’s office doesn’t use Quest, ask them to call the QuestSelect™ Lab Line on your card to pick up your test sample
• After your appointment, if you learn that your lab work did NOT go to Quest for processing, call the QuestSelect™ Lab Line for help. They’ll contact your doctor about getting future tests sent to Quest
• Access your results anytime, anywhere through MyQuest™ online
